Only APC applications and Non-Practising Declarations are done online – these do not require any hard copy forms.

New Zealand Graduate RegistrationUse this form if you completed a New Zealand Dietetic Qualification.
A Criminal Conviction Check must be completed and THIS FORM must accompany your application.
Recognition of Overseas QualificationsUse this form if you trained overseas and wish to have your dietetic qualifications recognised in NZ.
Overseas Trained RegistrationUse this form if you trained overseas and have now passed the Boards Registration Examinations.
APD Mutual Recognition RegistrationUse this form if you are an APD and are applying for Registration through Mutual Recognition.
APD applicants must successfully complete the MauriOra Foundation Cultural Competence Course, a copy of the certificate must accompany your application.
Board Registration Oral ExaminationUse this form if you have had your overseas qualifications recognised and passed the written examination.
APC ApplicationThis is completed online through the Dietitians Portal.
Return to Practice ApplicationUse this form if you are returning to practice after a break of more than 2 years or last held an APC more than 2 years ago.
Request to cancel entry in RegisterUse this form if you wish to cancel your entry on the Register.
Restoration to the Register (s.142) If your entry has been cancelled under s142 of the HPCA Act (2003) use this form if you wish to restore your name to the Register.
