Online Feedback Forms

The Dietitians Board now offers both members of the public and registered dietitians the ability to provide feedback online.

We offer two types of online forms: general feedback and report a problem.

General Feedback

General Feedback – for suggestions, compliments and complaints about the Board and the services we provide. You can opt to provide your contact details or remain anonymous. Please note, if you do not provide your contact details we are not able to respond or reply to you advising you of the outcome of your feedback.

Report a Problem

Report a Problem – for you to provide details of any issues you are experiencing with our online systems. This might be our public website or an issue in your secure Practitioner Portal. Reporting a problem enables us  to identify and rectify system issues, to improve the overall online experience for all users. If you are experiencing an issue online that needs urgent attention, such as inability to renew your APC online, please contact the Board’s office directly and we will aim to rectify your  issue as quickly as possible.

Additional Information

Please note, these feedback forms are not contact forms. For direct communication, to raise urgent matters, a complaint against a practitioner or to submit a Notification of Competence, please direct all communications to the Registrar, either by phone or email. Please do not use the general feedback facility.

The Board will review a summary of feedback received at each Board meeting to address matters raised.

Please note: The feedback forms are securely encrypted by Google Forms and your responses can only be viewed by Dietitian’s Board staff (or our Software Support team if required). However, please do not provide sensitive information such as credit card details or passwords in these forms.
