Frequently Asked Questions - the Audit Process

  1. Who can I ask for help?
    Please contact the Resource person in your area - names and e-mail contacts are listed on the previous website page.  Otherwise please e-mail the Registrar who will assist you find a mentor.

  2. I am involved in the next audit - what date do I need to send my documents in from?
    The legal requirement from 18 September 2004 when the HPCA Act became law, is a minimum of ten credits per year. Documentation will be required from 18 September 2004 until your current audit date, (either April or October).

  3. What type of evidence would help support my credits?
    Outcome evidence such as power-point presentations, summary pages, course outlines, resources developed and feedback from others are useful. As well, short reports of workshop or conference sessions can be used to list points important for contributing to new knowledge and skills you acquire. 

  4. Do I need to send in evidence for everything I have done?
    Evidence only needs to be submitted for credits 3, 4 and 5. No evidence is required for less than three credits.

  5. What activities constitute a practice review?
    Practice review activities are intended to have a peer review component.  This could include feedback from colleagues on changes made to policies and procedures documentation, as well as peer review of clinical case notes, clinical consultations or teaching sessions.  Evidence could be a signed report from the peer reviewer.

  6. How many credits do I need to send in for audit?
    You are required to send in fifteen credits per year with a minimum of ten credits per year, or on a pro-rata basis. This means a minimum of seventy five credits(75) over five years and a maximum of one hundred fifty credits (150) over five years.
